What to bring to your first appointment

To make things go smoother at your first appointment, please bring along your family doctor’s information, a list of medications, health conditions, and medications, and your dental insurance policy booklet and card.

Financial policy

We require all patients to pay for services rendered on the day of treatment. If you require payment arrangements, please discuss them with us prior to your treatment.

Insurance

Your dental insurance is a contract between you and your insurer. You are responsible for all dental fees, including any fees not covered by your policy. We can direct bill to your insurance provider.

Combat Veterans

At Spearmint Dental we will not charge any out of pocket costs for our Combat Veterans with a valid I.D.
